Mike Cushing Memorial Scholarship

REQUIREMENTS CHECKLIST
IN ORDER TO BE CONSIDERED FOR THE SCHOLARSHIP, ALL OF
THE FOLLOWING ITEMS MUST BE SUBMITTED WITH YOUR APPLICATION:
  1. A typed essay of 400-500 words on one of the following two topics:
    • What education or training would reduce or eliminate the demand for illegal drugs?
    • Drug problems in schools and its impact on the community
  2. Completed applicant information form.
  3. Completed school and community activities/honors for the applicant.
Applications must be returned by mail, and postmarked

NO LATER THAN DECEMBER 31, 2006

Applications postmarked after this date will not be considered.

Judging will be handled by the IDEOA Scholarship Committee. We look forward to receiving your completed application.
